Summary: Serving as a key member on each prospective team. Provides initial and ongoing outreach and support to potential and current clients on respective teams. Serves as intensive Case Manager to support, teach, advocate, empower individuals with MH and/or co-occurring disorders; including developing person centered recovery goals.
Essential Responsibilities and Target Outcomes:
• Builds trusting, recovery orientated rapport with individuals with mental health & cooccurring disorders and can assist with linking to team.
• Provides input and advocacy by actively participating in clinical team meetings.
• Selectively utilizes personal recovery experience to provide hope and empowerment to assist & motivate.
• Demonstrate and role model effective skills to lead towards recovery.
• Engages clients within clinic and community to assist with navigation throughout the agency. • Facilitates client groups based on topics relevant to the population.
• Produces concise documentation regarding on-ongoing services with client; including progress and recommendations.
• Work with family members by providing education and support as needed/if applicable.
• Works with client to achieve and maintain skills to produce an independent and selfsustaining life; including assistance with finances and entitlements, support with healthy activities of daily living skills and supervision of medications
• Performs other related duties as assigned.
Minimum Requirements: E
Education: High School Diploma or G.E.D
Credentials and Licenses: CCAR and/or Advocacy Unlimited and/or Recovery Support Specialist Certification
Experience:
• Successful related work and/or life experience that is relatable to client needs
• Beginner to intermediate experience with Microsoft Office products including Word and Outlook. • Experience in, or the ability to learn how to use an Electronic Medical Record (EMR) system Competencies:
• Effective interpersonal skills, ability to problem solve, use good judgment and work independently as well as collaborative work within a team setting
• Flexibility and ability to adapt creatively to stressful situations including agility to work in a fast pace environment.
• Awareness/sensitivity to all types of issues related to client diversity.
• Strong organizational abilities and verbal and written communication skills.
• Ability to ensure the security and confidentiality of client information and records in a manner consistent with professional and CMHA codes of ethics.
$18-23 per hour
