Peer Navigator IMPACT

Summary:
  • Provides initial and ongoing outreach and support to homeless/at risk individuals as well as potential and current clients.
  • Engages in traditional and recovery-oriented case management tasks including, but not limited to, the coordination of related services such as housing, budgeting, skill building, etc.
  • Goal is to empower and educate clients to rediscover their ability to pursue and achieve a higher quality of life that allows them to prosper in their community of choice.
  •  Builds trusting, recovery-oriented rapport with homeless/at risk individuals experiencing Opioid Use Disorder to encourage their participation in Medication Assisted Treatment (MAT). 
  • Provides input and advocacy by actively participating in IMPACT and community meetings. 
  • Visits local shelters and encampments to disseminate program/MAT information. 
  • Coaches and assists clients with all homeless challenges including, but not limited to, personal hygiene, nutrition, community integration, and service/resource navigation. 
  • Draws upon personal recovery experience when appropriate to provide hope and empower clients. 
  • Assures timely documentation of client progress and works with managers and IMPACT team to develop recommendations for future services in an effort to make the client self-supporting. 
  • Incorporates “WRAP” plans and philosophy into daily work. 
  • Performs other related duties as assigned.

Experience:
  • Successful related work experience, preferably a minimum of 1-2 years.
  • Lived experience and at least two years of sobriety from OUD.
  • Beginner to intermediate experience with Microsoft Office products including Word and Outlook.
  • Experience in, or the ability to learn how to use an Electronic Medical Record (EMR) system which includes appointment scheduling, authorization tracking, assessments, treatment plans, progress notes and discharge summaries.

Salary range: $18 to $22 per hour
