**1st shift 8am-4pm, Sat and Sun
Summary: Provides one on one support by assisting identified client with achieving daily living skills and routine activities/chores. Provides one on one coverage to the identified client as specified in client’s daily schedule. Assist identified client with maintaining all outside appointments and activities by providing transportation and reporting back to program staff clients’ accomplishments and information related to client care.
Essential Responsibilities and Target Outcomes:
• Provides direct assistance to identified client in the accomplishment of activities of daily living, routine program chores, and personal treatment goals.
• Demonstrates and coaches the development of skills identified as goals in the treatment plan.
• Provides transportation for identified client to maintain medical or other appointments outside the program.
• Develop weekly schedule of activities that identified client and Life Coach will engage in throughout week. Will review schedule with client’s primary staff person.
• Responds appropriately and in compliance with agency Policies and Procedures to emergent incidents/accidents that develop with client and/or in the environment of care by contacting and reporting to 911 and chain of command.
• Engage identified client in community integration by participating in local activities with client (e.g. Library, attending community outings and events).
Communicate with identified client’s primary staff person any changes in mental status or issues that occurred during scheduled shift with identified client.
• Completes all shift notes and required documentation that relate to client care, and communicates to incoming shift staff all relevant activity.
• Maintain safety, security and satisfaction of identified client by monitoring environment of care and ensuring physical environment is in compliance with regulations, and by making and documenting routine hourly checks of clients.
- Perform other related duties as assigned.
Minimum Requirements:
Education: High School Diploma or GED.
Credentials and Licenses:
- CPR and First Aid Certification preferred. Valid Driver’s license.
- Will become certified in MANDT, First Aid, and CPR.
- Participate in trainings on an annual basis, as assigned.
Experience:
- Successful direct care experience in relevant field/setting with Mental Health Population, preferably a minimum of 1-2 years.
- Beginner to intermediate experience with Microsoft Office products including Word and Outlook.
- Experience in or the ability or willingness to learn how to use and Electronic Medical Record (EMR) system or the ability and willingness to learn how to use one, which includes appointment scheduling, authorization tracking, assessments, treatment plans, progress notes and discharge summaries.
$17-19 per hour
