POSITION: Patient Navigator 
RESPONSIBLE TO: Call Center Manager 
Pay Grade:  17.00-19.55
 
SUMMARY: This position oversees the implementation and continued development of a patient navigation program that helps guide the patient through the health care system, works to overcome obstacles faced by the patient receiving the care and services they require, including connecting them to funding opportunities, alternate resources, care inside and outside of PPRM, and managing logistical needs of accessing care. Given the nature of our environment, the navigator will have a particular focus on patients seeking abortion services.  


QUALIFICATIONS:

  • High school diploma or equivalent.  
  • Previous experience with Planned Parenthood or patient navigation preferred.  
  • Must be committed to the goals, philosophies, and mission of Planned Parenthood of the Rocky Mountain (PPRM) 
  • Excellent organizational skills and great attention to detail 
  • Ability to work on under pressure and on tight deadlines 
  • Ability to provide effective case-management to a varying caseload of patients

ESSENTIAL FUNCTIONS:

  • Work with patient to identify needs and connect patients to appropriate resources and supports.  
  • Schedule patient for care at PPRM or connect them to care with an outside entity as needed.  
  • Arrange legal counsel, transportation, and travel logistics, dependent care, or anything else necessary to aid the patient in accessing care.  
  • Facilitate communication between the patient, support person(s), and healthcare providers to ensure patient satisfaction and quality of care. 
  • Inform the patient on financial aspects of care and link the patient to financial resources as needed. 
  • Partner with operational leaders including legal, compliance, contracts, and development to initiate, modify and/or terminate funding relationships.  
  • Create systems for documenting and tracking fund usage. 
  • Partner with accounting to create systems for requesting reimbursement from each fund and ensure proper allocation once invoices paid 
  • Create and disseminate training materials for Health Center, call center, and agency staff who intersect with the patient navigation program. 
  • Develop processes for auditing compliance with fund usage, various state laws, PPFA requirements. 
  • Develop and oversee systems for when, how and if funds should be used, in what order, and with what discretion. 
  • Manage technology in a way that streamlines communication with patients needing navigation. Partner with operational leaders to develop solutions not already being utilized.  
  • Manage programmatic oversight and coordination of external funding and grant resources, including Wyoming Fund, Justice Fund, etc. 
  • Act as Subject Matter Expert for community resources for patients 
  • Identify opportunities for new and expanded community partnership; in partnership with manager and director, act as liaison between PPRM and community partners when appropriate.  
  • Accommodate interpretive language services when needed. 
  • Facilitate pre-and post-abortion visits and ensure continuity of care and linkage to community resources. 
  • Collect, track, and report required program data. 
  • Provide nonjudgmental education to patients by offering information and/or referrals on reproductive and sexual health issues. 
  • Completes all responsibilities according to established protocols, policies, and standard practices in the areas of customer service, quality assurance and regulatory compliance programs such as HIPAA (Health Information Portability & Accountability Act), OSHA (Occupational Safety & Health Administration) and CLIA (Clinical Laboratory Improvement Act) 
  • Communicate directly with patients via phone, mail, and Patient Portal 
  • Demonstrate excellent customer service to both internal and external customers by working towards a positive outcome with any problems encountered 
  • Document accurately, appropriately, and efficiently all telephone interactions with patients and complete, accurate and timely documentation 
  • Demonstrate ongoing proficiency in Electronic Health Records (EHR) and Electronic Plan Management (EPM) systems 
  • Provide follow-up communication to patients regarding follow up plans and lab results as related to the care received at PPRM  
  • Participates in abortion services (participation can include referrals, information, education, counseling, performing, scheduling, etc.). 
  • Other duties as assigned by supervisor. 
  • COVID-19 Vaccine: PPRM is joining our partners in public health across our region and county, as well as many other Planned Parenthood affiliates, by requiring the COVID-19 vaccine for all staff.
