Clinical Therapist

 Fellowship Health Resources affiliated with Elwyn in 2018 and provides behavioral health services to improve the quality of life for individuals living with mental illness and addictions.

  1. GENERAL RESPONSIBILITIES:
To provide leadership, individual and group counseling in therapeutic setting designed to serve young adults through geriatric clients with mental illness to achieve social, economic, emotional, and personal independence in one or more program(s).

II. REQUIRED QUALIFICATIONS WITH REASONABLE ACCOMODATION:

A. PRE-EMPLOYMENT EDUCATION:

 
Master's degree in counseling rehabilitative counseling, social work, clinical psychology or closely related discipline. A Clinician shall be a person who has a doctoral or master's degree in psychology, counseling, social work, nursing, rehabilitation or closely related discipline from an accredited college or university, or an RN with a certificate in mental health nursing from the American Nurses Association. Licensure preferred (LPC,LCSW).
 

B. PRE-EMPLOYMENT EXPERIENCE:

A commitment to community based mental health services. Knowledgeable in the principles and practices of therapeutic communities. A minimum of two years counseling experience working with chronic mentally ill adult population. Thorough
 
understanding of the behavioral aspects of mental illnesses, symptoms, and social work counseling techniques. Previous experience with co-occurring mental illnesses required. State driver's license and record in good standing. Computer literacy

Demonstrating a willingness to participate as a "team" member to improve the overall effectiveness of one's work group and to achieve group goals.

III. ESSENTIAL FUNCTIONS AND PERFORMANCE STANDARDS:

 
  1. *Maintains a clinical chart, which is accurate, up-to-date, and clinically sound. Documents in the chart and the CARe system at a clinical level commensurate with education. Produces clear, timely, and accurate record-keeping in compliance with Program, Department, State and OSHA regulations on daily or as needed
 
  1. *Develops treatment plans which: 1) clearly describes problem, states goals, states measurable objectives, and states treatment
 
  1. *Makes clinically sound observations and provides vital clinical information in case conferences and treatment planning
 
  1. *Develops and maintains clinically sound and therapeutically valid relationships with clients.
 
  1. *Has the necessary insight and clinical judgment to respond effectively to emergency situations.
 
  1. Assists in monitoring treatment-planning activities to ensure that appropriate needs assessment, information review, medical/physical history and other pertinent information is received and evaluated. Participates as an active team member in the development and implementation of Program treatment plan
 
  1. Assists Director in monitoring treatment delivery
 
  1. Knowledge of prescribed medication used in the program and its
 
  1. Fosters positive relationships with both primary customers (those who use our service) and supporting customers (members of the community, regulatory agencies, ).
 
  1. Attends scheduled staff meetings. In staff meetings applies education, years of specialized training and experience to assist in goal setting and treatment
 
  1. May provide consumer transportation as needed if applicable to
 
  1. Promotes group process, involvement, and community
 
  1. Provides advocacy for
 
  1. Attends scheduled supervision with Director. Notifies supervisor of any pertinent information to ensure quality client
 
  1. Completes assigned billing in required time
 
  1. Integrate the PRISMMODEL for effective treatment of serious and persistent mental illness,
into everyday practice and clinical intervention.
 
  1. Regularly reads FHR emails to stay abreast of pertinent work
 
  1. Perform any other duties and/or tasks that may be assigned to him/her on an as needed
 

IV. WORKING CONDITIONS:

 
  1. Necessary Travel: Occasional driving required in the course of
 
  1. Unusual Work Hours: Scheduled hours may be required in evenings, weekends, or overnights. Beeper coverage may be applicable. Ability to respond to crisis situation as presented. Scheduled holiday and severe weather attendance may be
 
  1. Unusual Environmental Conditions: Work area - inside and outside. Environment may be noisy at times. Hazards - Potential for exposure to infectious disease. Necessity to practice to Standard Precautions at all
